A filler nodule is a firm, localised lump that can be felt or seen after a dermal filler injection. Some are simply product that was placed too superficially or in a clump; others are the body's inflammatory or immune reaction to the filler, sometimes appearing weeks or months later. The type decides the management, so a lump is examined before anything is injected or dissolved.
A non-inflammatory lump of hyaluronic acid can often be dissolved with hyaluronidase. An inflammatory or delayed-onset nodule may need a different sequence, and consensus guidance advises against simply massaging or dissolving before the cause is understood (Signorini, Plast Reconstr Surg 2016; Convery, J Clin Aesthet Dermatol 2021). Non-hyaluronic fillers such as calcium hydroxylapatite or PLLA do not dissolve with hyaluronidase at all.
Under the eye, where the skin is thinnest, nodules are the most common reason people seek a second opinion. The full guide: under-eye filler lumps and nodules; for lumps that appear later, delayed lumps after filler.
Some early swelling settles within days, but a true nodule often persists; MRI studies have shown hyaluronic acid filler remaining for years (Master 2024). An examination decides whether it needs treatment.
If it is hyaluronic acid and not actively inflamed, usually yes, with hyaluronidase. Ultrasound can show where the product sits so it can be targeted.
